MDPCP Practice Applications Open Aug 1 - 31

Posted almost 6 years ago by Elaine Crain

Online applications for practices to apply to participate in the Maryland Primary Care Program (MDPCP) will open next Wednesday, August 1, and will remain open until Friday, August 31.

Governor Larry Hogan, together with the Maryland Department of Health (MDH) and the Centers for Medicare and Medicaid Services (CMS), recently signed and enacted Maryland’s Total Cost of Care All-Payer Model, known as the “Maryland Model”. The announcement also included approval of the MDPCP, which will launch Jan. 1, 2019.

The MDPCP is a voluntary program for primary care practices that provides financial and technical support to assist in the transformation to “advanced primary care.” Primary care has always been the foundation of care in Maryland. With this program, Maryland’s primary care providers can play an increasingly important role in improving health outcomes, while reducing unnecessary emergency department and hospital visits.

Eligible primary care providers include physicians, clinical nurse specialists, nurse practitioners, and physician assistants in a primary care practice with a variety of specialty designations. Specialties include General Practice, Internal Medicine, Geriatric Medicine, Family Medicine, OB/GYN, and co-located Psychiatry.

To help practices prepare for the application, the Maryland Department of Health will be hosting a Practice Opportunity and Program Review Webinar this Thursday, July 26th, from 5-6pm. The presentation will provide an overview of the program, discuss the opportunities for practices, and provide a sneak peek at how to log into and navigate the online application portal.

We encourage interested primary care practices to begin preparing for the application in advance.  Official requirements are available in the Request for Applications (RFA). For additional information, please view the practice resources on the MDH website. Questions may be sent to MarylandModel@cms.hhs.gov.
